How THC-O acetate is made
A processor starts with a THC base, usually delta-8 THC distilled from hemp, and reacts it with acetic anhydride. The reaction adds an acetyl group to the molecule. The acetyl group has to be removed by the body before the THC becomes active, which is the leading explanation for the slow onset and long duration users report. The same chemistry means unreacted reagents and solvent residue can carry into the final oil if the batch is not purged and tested.

The FDA has also said it has not approved any hemp-derived cannabinoid for food, supplement, or medical use.

Risks and open questions
There are no controlled human trials on THC-O acetate. Most of what circulates online comes from user reports. Two concerns stand out. First, the conversion process uses acetic anhydride and other reagents, so a poorly purged batch can carry residues. That is what contaminant testing is for. Second, heating acetate esters can, under some conditions, produce ketene, an irritant gas. The CDC tied vitamin E acetate to the EVALI outbreak, which shows that acetate additives in vape oil can harm lungs. Whether THC-O acetate forms ketene at normal vape temperatures is not settled, so inhaling it carries unknown risk.
Other reported issues include fast tolerance, anxiety, a racing heart, nausea, and a hangover that lasts into the next day. Daily use of any high-potency cannabinoid raises the risk of dependency and worsening anxiety.
