The short answer
Cannabis can change how your body handles other drugs, and the risk runs in both directions. CBD is the bigger troublemaker. It shuts down CYP3A4 and CYP2C19, two of the busiest liver enzymes for clearing prescription medicines. THC is mostly a passenger that gets chewed up by those same enzymes, so anything that blocks them makes THC hit harder and stick around longer. If you take a blood thinner, a seizure medicine, an opioid, or a drug with a narrow safety margin, that is a conversation to have with a pharmacist before your first dose, not after.
Why a plant changes prescription drug levels
Most medications leave the body through a family of liver enzymes called cytochrome P450. CBD inhibits CYP3A4 and CYP2C19, and has weaker effects on CYP2C9 and CYP2D6. When those enzymes slow down, the other drug stays in your bloodstream longer and at higher concentrations. Think of it as grapefruit juice, except CBD usually gets taken every day, so the brake stays on instead of fading within an hour.

THC goes the other way. It is broken down by CYP3A4 and CYP2C9, so any drug that blocks those enzymes lets THC linger. That list includes the HIV drug ritonavir and some antifungals. CBD competes for the same enzymes too, which is one reason a high-CBD product can feel stronger than its THC percentage suggests.
Where the risk is highest
Seizure medicines
This is the best documented interaction. In trials of the purified CBD drug Epidiolex, adding CBD to clobazam raised levels of clobazam's active metabolite and caused heavy sedation. The label tells prescribers to consider lowering the clobazam dose. CBD plus valproate has been linked to liver enzyme rises and lower platelets.

Blood thinners
Case reports describe INR climbing in people on warfarin after they added CBD. Anyone on warfarin who starts a cannabinoid should expect more frequent INR checks. Direct oral anticoagulants such as apixaban and rivaroxaban are partly CYP3A4 substrates as well, though the human data there is thinner.
Drugs with a narrow margin
Transplant medicines (tacrolimus, cyclosporine, everolimus), some antiarrhythmics, and certain chemotherapy agents leave little room for error. A modest rise in blood level can move a drug from helpful to toxic. I would not stack high-dose CBD with these without a prescriber watching levels.
Sedatives, opioids, and alcohol
This one is not about enzymes at all. THC and CBD add to the drowsiness and breathing depression caused by benzodiazepines, opioids, sleep aids, muscle relaxants, and alcohol. Mixing opioids with cannabis is a known factor in overdose deaths.
Antidepressants and antipsychotics
Several SSRIs and antipsychotics share CYP2C19 and CYP3A4, so CBD can nudge their blood levels. Inhaled cannabis smoke is a separate issue: smoke induces CYP1A2, which lowers clozapine and olanzapine levels. Stop smoking suddenly and those levels can climb into toxic range. Prescribers often adjust doses when a patient quits.
Heart and blood pressure medicines
THC raises heart rate and can drop blood pressure, which stacks with antihypertensives and nitrates. CBD may raise levels of calcium channel blockers and statins that run through CYP3A4, which matters if you notice new muscle aches or dizziness.
HIV and hepatitis C antivirals
Ritonavir and cobicistat are strong CYP3A4 inhibitors, so they push THC and CBD levels upward. Some regimens interact in the other direction too, which makes pharmacist review worthwhile.
CBD and THC are not interchangeable here
Almost every well-characterized interaction involves CBD, and usually at doses of 200 mg a day or more, the range used in epilepsy trials. A 10 mg CBD gummy is not the same exposure. THC carries its own risks, mostly additive sedation and heart rate effects, but it is a weaker enzyme inhibitor. That distinction matters when someone tells you their tincture is "low dose" without knowing the actual milligram count.
Spacing doses apart does not fix this
People often ask if taking cannabis two hours before or after their pills solves the problem. It does not. Enzyme inhibition is a whole-body effect that lasts as long as CBD is in your system, which with daily use can be several days. Timing helps with stomach upset from a single dose, not with metabolic interactions.
What to do before you start
- Write down every medication, including over-the-counter drugs and supplements. Acetaminophen, ibuprofen, and herbal products all count.
- Bring that list to a pharmacist and say plainly that you are considering medical cannabis. Pharmacists see interaction data all day and most are comfortable discussing this.
- Start with the lowest effective amount and change one thing at a time, so you can tell what caused a new symptom.
- Watch for unusual drowsiness, confusion, dizziness, easy bruising, or a racing heart, and report them.
- Never stop a prescribed medication on your own because you read about an interaction.
