The short answer

Start with 1 to 2.5 mg of THC, hold that dose for two or three days, then step up by the same amount. Most people land somewhere between 2.5 and 10 mg per dose, but that range is wide for a reason: tincture hits people differently depending on the ratio, the base oil or alcohol, and whether you hold it under your tongue or swallow it. Write down what you take and when. Guessing is how people end up on the couch for six hours.

Medical Cannabis Tincture Dosing: Milligrams, Drops, and Timing

Why tinctures are the easiest place to learn

A tincture is liquid, which means it measures. A gummy is one size. A vape puff is a guess. With a dropper or an oral syringe you can hit 0.25 mL the same way twice, and that consistency is the whole game when you are trying to find your floor. Treat the first bottle as a measuring stick, not a supply.

Read the label before you open it

Two numbers matter: total THC in the bottle and milligrams per milliliter. Labels in legal US markets usually list both, though not always in the same spot. Divide total milligrams by total milliliters and you have your per-mL strength.

The only math you need

A 30 mL bottle with 300 mg of THC is 10 mg per mL. One full dropper (1 mL) is 10 mg. If you want 2.5 mg, you want 0.25 mL. If your dropper only has marks for half and full, buy a 1 mL oral syringe at any pharmacy. They cost a couple of dollars and the 0.1 mL lines make small doses possible.

Some tinctures are much weaker, especially CBD-heavy ones, so run the division every time. A "1000 mg" bottle could be 1000 mg of CBD and 20 mg of THC, which is a very different product from what the front label suggests.

Pick a starting dose

  • Never used THC: 1 to 2.5 mg, once.
  • Use THC but not by mouth: 2.5 mg, and do not repeat for at least four hours.
  • Regular edible user: start at half of what you think you need, especially with a new brand.
  • CBD-dominant product: CBD doses often run 10 to 50 mg, and CBD has its own curve. Small amounts of THC in a CBD tincture still count toward your total.

Sublingual or swallowed

This choice changes everything. Hold the tincture under your tongue for 60 to 90 seconds and you feel it in 15 to 45 minutes. Swallow it right away and you are waiting one to three hours, because the THC goes through your liver first and converts into 11-hydroxy-THC. That route hits harder, lasts longer, and varies more from person to person. Pick one and stay with it while you titrate, otherwise you are changing two variables at once.

A titration schedule that holds up

  1. Days 1 and 2: 1 to 2.5 mg, same time of day, same method.
  2. Days 3 and 4: if nothing changed, add 1 to 2.5 mg.
  3. Keep stepping every two or three days until you get relief or side effects.
  4. If you get anxious, foggy, or too sedated, drop back one step and stay there.
  5. Never double the last step. Small jumps, longer holds.

What shifts your number

  • Other medications. Cannabis interacts with drugs processed by CYP450 enzymes, including warfarin, clobazam, tacrolimus, and some antidepressants.
  • Food. A swallowed dose on an empty stomach can land faster and harder than the same dose after a meal.
  • Liver function and age. Both slow clearance and raise the odds of a longer ride.
  • Alcohol and sedatives. They stack with THC on drowsiness and coordination.
  • Daily use. Tolerance builds fast, and so does the urge to chase it. Take days off when you can.

Keep a log, even a sloppy one

Time, dose in mg, method, food, and how you felt at one hour, two hours, and four hours. Three weeks of notes tells you more than any dosing chart. It also gives your clinician something concrete to work with at your next appointment.

If you take too much

It passes. THC will not stop your breathing the way opioids can, but it can make you feel panicky and unsteady for a few hours. Sit somewhere safe, drink water, do not drive, and do not take more to "fix" it. Note the dose that did it, because that number is your ceiling for a while.

Storage and the boring basics

Keep the bottle closed, upright, and out of sunlight. Alcohol-based tinctures hold up for years; oil-based ones fade faster, so buy sizes you will finish. Lock it up if there are kids or pets in the house, and write the date you opened it on the label.

None of this replaces a conversation with your recommending clinician, especially if you take prescription medication. Bring the label and your log to that visit. It makes the conversation shorter and better.
