Pick the product that fits your condition, your clinician's target dose, and a lab report you can read. Brand name, packaging, and bottle price come after that.

Start With the Dose, Not the Bottle

CBD dosing is not one size fits all. Trials of purified CBD for seizures used hundreds of milligrams per day under medical supervision. Most people buying at a dispensary start somewhere near 10 to 25 mg. The number that matters on a tincture is milligrams per milliliter, not the total on the front of the bottle. A 1,000 mg bottle holding 30 mL works out to about 33 mg/mL, so a full dropper is a 33 mg dose. If you want to start at 10 mg, that is roughly a third of a dropper, which is hard to measure without a marked syringe. Buy products that include one.

Ratio and Spectrum: Decide How Much THC You Can Handle

Medical products are sold as CBD-dominant ratios like 20:1, 10:1, or 1:1. Full spectrum oil keeps trace THC and other cannabinoids. Broad spectrum removes THC. Isolate is CBD alone. Plenty of patients say full spectrum works better for them, though the research behind that claim is still thin. What is not thin is the THC. A 20:1 tincture can still carry enough THC to show up on a workplace drug screen after daily use. If you get tested, ask for a product whose COA shows THC not detected, or talk with your clinician first.

Ask for the Certificate of Analysis

Legal state markets test products, and hemp items sold online vary a lot. A certificate of analysis from an accredited lab should list cannabinoid potency plus pesticides, residual solvents, heavy metals, microbials, and mycotoxins. Match the batch number on the COA to the batch number on your package. If the shop cannot produce one, that is your answer. Labels also drift over time, so I look for a COA dated within the past year. Cannabinoids break down with heat and light.

Delivery Method Sets Onset and Duration

Sublingual tinctures kick in within 15 to 45 minutes and last a few hours. Edibles take 30 to 120 minutes and can run six hours or longer, which makes them easy to overdo. Inhalation works in minutes and fades faster. Topicals stay local and do not reach the bloodstream in meaningful amounts. Capsules land near edibles. For medical use, most people do best with one method they can repeat at the same time each day. If you vape, buy from licensed sources. Vitamin E acetate in unregulated cartridges was tied to the 2019 lung injury outbreak.

Compare Cost Per Milligram

Divide the price by total milligrams of CBD. A $60 bottle with 1,500 mg costs 4 cents per mg. A $45 bottle with 300 mg costs 15 cents per mg. Cheap is not a deal when the COA is missing, and high CBD on the label does not always mean a lower cost per dose.

Interactions and Safety Checks

CBD blocks some liver enzymes in the CYP450 family, much like grapefruit does. That can raise blood levels of clobazam, warfarin, and certain anti-seizure drugs. Tell your pharmacist about every cannabis product you take, topicals included. Drowsiness, dry mouth, and loose stools are the most common complaints, and they tend to show up when doses climb too fast.

What to Ask Before You Buy

  • How many mg of CBD per mL or per unit?
  • Is there a batch-specific COA, and can I see it?
  • What is the THC percentage, and will it build up with daily use?
  • Which delivery method fits the timing I need?
  • What is the cost per milligram, not per bottle?
