Short answer
CBD does not show up on a THC blood test. Standard drug panels target delta-9-tetrahydrocannabinol (THC) and its metabolite 11-nor-9-carboxy-THC (THC-COOH). CBD is a different molecule with different metabolites, such as 7-COOH-CBD. Labs do not look for those on a routine panel.

What a THC blood test measures
- Immunoassay screen: a fast test that flags THC-COOH. The federal urine cutoff is 50 ng/mL. Blood cutoffs vary by lab.
- Confirmatory test: gas or liquid chromatography with mass spectrometry (GC-MS, LC-MS/MS). This step separates CBD from THC by molecular weight.
- Blood draw timing: THC peaks within 10 minutes of smoking and clears in about 4 hours. THC-COOH stays in blood for 1 to 3 days.
Three paths to a positive THC result
- THC in the product. A 30 mL bottle of oil at 0.3% THC holds about 80 mg of THC, or about 2.7 mg per 1 mL serving.
- Mislabeled products. In a 2017 JAMA study of 84 CBD products, about 1 in 5 contained THC.
- Dose and frequency. THC-COOH stores in fat tissue. Daily doses build a reserve and stretch the detection window.

What is not settled
Research on CBD-to-THC conversion is mixed. Lab work with simulated stomach acid showed conversion at low yield. Human trials with pharmaceutical CBD found no THC metabolites above cutoff in most samples. Case reports exist of positive screens after CBD use, and confirmatory tests in those cases showed THC-COOH at trace levels. The THC source in those reports is not always clear.
